STEP 1:

Adult and/or Dependent (15 years of age and under)
NOTE: For dependents, you will need a separate form for every dependent you are applying for.
Click on the button(s) and download the application form.
Once the form opens, select file, select save or save as and choose a location on your computer desktop to save it to.
This step is very important as this form contains special components that will only work if saved onto your computer desktop.
STEP 2:
Read the application instruction page with your downloaded form for detailed instructions and a list of required documents to submit with your form.
Completely fill out the form(s) and save it in the same location on your desktop.

Email completed form(s) with ALL required documents to DCBIPS@hotmail.com
